    Efficacy and safety evaluation of percutaneous vertebroplasty in treatment of osteoporotic vertebral compression fractures

    Objective To evaluate the efficacy and safety of percutaneous vertebroplasty in the treatment of osteoporotic vertebral compression fractures.Methods From August 2016 to May 2019, a total of 84 patients with osteoporotic vertebral compression fractures treated in our hospital were enrolled in the study, and divided into traditional treatment group and percutaneous vertebroplasty group by random number table method.Traditional non-surgical method was administered to patients in the traditional treatment group (n=42), and percutaneous vertebroplasty was performed in the percutaneous vertebroplasty group (n=42).The differences in VAS pain score, vertebral compression rate, Cobb angle of injured vertebral body, and ADL score of daily living activities between the two groups were compared.Results After treatment, the VAS pain score (1.48±0.15), vertebral compression rate (16.58±7.15), Cobb angle [(9.23±3.11)°], and ADL score (21.08±0.45) in the percutaneous vertebroplasty group were significantly lower than those in the traditional treatment group (P<0.05).Conclusion Compared with the traditional conservative method, percutaneous vertebroplasty is more effective and safer in the treatment of osteoporotic vertebral compression fractures, as it can significantly alleviate patients’pain degree and improve their daily living activities, thus it is worthy of clinical application.

    Analysis on efficacy of micro-plate and screw internal fixation for metacarpal fracture

    Objective To investigate the effect of micro-plate and screw internal fixation on metacarpal fracture.Methods A total of 80 cases (89 fractures) of metacarpal fractures treated with micro-plate and screw internal fixation from January 2016 to December 2018 were selected in the study, and their clinical data were retrospectively analyzed.Results The average hospitalization duration was (5.12±1.06) d; the average follow-up time was (11.36±3.95) months; the average fracture healing time was (43.6±3.7) d; the excellent treatment rate was 95.51%; the average overall degree of metacarpal joint motion was (236.84±8.06)°; the average ratio of grip force loss in the affected to unaffected side was (2.61±0.21) %; the total incidence of complications was 7.50 %.Conclusion Micro-plate and screw internal fixation for the the treatment of metacarpal fracture has a good effect and prognosis, with a low incidence of complications and a high level of safety.

    Objective To study the clinical effect of reconstruction of metacarpophalangeal joint defect with semi-hook carpometacarpal joint surface transplantation.Methods Eighty-two patients with metacarpophalangeal joint defects admitted to our hospital from May 2014 to November 2018 were enrolled and randomly divided into control group (40 cases) and observation group (42 cases) by random number table.The control group was treated with traditional metacarpophalangeal arthroplasty, while the observation group was treated with semi-hook carpometacarpal joint surface reconstruction surgery.The clinical efficacy, recovery time, treatment cost, and incidence of postoperative complications of the two groups were compared.Results After treatment, the total clinical effective rate of the observation group (95.24%) was higher than that of the control group (77.50%), the recovery time was faster, the treatment cost was higher, and the postoperative complication rate (2.50% vs 17.50%) was lower (P<0.05).Conclusion Reconstruction of metacarpophalangeal joint transplantation with semi-hook carpometacarpal joint can accelerate recovery, and has fewer postoperative complications.Its significant clinical efficacy allows it to be popularized.

    Effect of laparoscopic hernia repair on inguinal hernia

    Objective To observe the effect of laparoscopic hernia repair on inguinal hernia.Methods A total of 171 patients with inguinal hernia who received surgical treatment in our hospital from January 2017 to December 2018 were divided into traditional surgery group (56 cases) and laparoscopic group (115 cases) according to operation methods, and their surgical effects were compared.Results The laparoscopic group had longer operational time, less bleeding amount, lower VAS score at 1 day after surgery, shorter recovery time and hospitalization time, and decreased postoperative complications and half-year hernia recurrence rate than the control group (P<0.05).Conclusion Laparoscopic hernia repair for inguinal hernia is safe and effective, with less bleeding amount, lighter postoperational pain, quicker postoperative recovery, and smaller postoperative complication rate and recurrence rate, despite of longer operation time.

    Comparative study of plasmakinetic resection of prostate and transurethral resection of prostate in treatment of benign prostate hyperplasia

    Objective To observe and compare the clinical effects of plasmakinetic resection of prostate (PKRP) and transurethral resection of prostate (TURP) in the treatment of benign prostate hyperplasia (BPH).Methods BPH Patients in TURP group (n=17) and PKRP group (n=18) were treated with corresponding operations.The intraoperative blood loss, indwelling time of urinary catheters, irrigation time of bladder, indwelling time of fistula tubes, postoperative hospital stay, maximum urinary flow rate (Qmax), and International Prostate Symptom Score (IPSS) were observed in the two groups.Results The intraoperative blood loss in the PKRP group was significantly less than that in the TURP group (P<0.01).Six cases received blood transfusion (200-600 mL) in the TURP group and none in the PKRP group.One patient aged 81 years in the TURP group died of heart failure due to excessive postoperative bleeding in the TURP group.Four patients didn’t have cystostomy in the PKRP group.Two patients in the TURP group had water poisoning after operation.There were no significant differences in indwelling time of urinary catheters, irrigation time of bladder, indwelling time of fistula tubes, and postoperative hospital stay between the two groups (P>0.05).No significant differences in Qmax and IPS were shown between the two groups three months after operation.Conclusion PKRP is more effective than traditional TURP in the treatment of BPH with fewer complications and significantly accelerated recovery of patients.

    Comparison of laparoscopic myomectomy, total hysterectomy, and subtotal hysterectomy for postoperative ovarian function/sexual function of patients

    Objective To compare the effects of laparoscopic myomectomy, total hysterectomy, and subtotal hysterectomy on uterine fibroids.Methods Patients with uterine fibroids who underwent laparoscopic myomectomy (n=40, group A), who met the indications for total hysterectomy (n=40, group B), and who met the indications for subtotal hysterectomy (n=40, group C) were selected from The Second People’s Hospital of Huishan District from May 2015 to April 2017.After six months of surgery, the changes of ovarian function and sexual function were compared among the three groups.Results Before surgery, there was no statistical difference in ovarian function among the three groups (P>0.05); after six months of surgery, the ovarian function of group A had no significant changes compared with that before surgery (P>0.05); the ovarian functions of group B and group C were lower than before (P<0.05), and there was no statistical difference between group B and group C (P>0.05).Before surgery, there was no statistical difference in the score of sexual function among the three groups (P>0.05); after six months of surgery, the score of group A was higher than the scores of group B and group C (P<0.05), and there was no statistical difference between group B and group C (P>0.05).Conclusion Laparoscopic myomectomy for patients with uterine fibroids has no significant effect on ovarian function, and can improve sexual function of patients.Under the premise of clear surgical indications, it can be the first choice for the treatment of uterine fibroids.

    Value of intraoperative ultrasound for patients with hepatic tumor resection

    Objective To explore the value of intraoperative ultrasound for patients with hepatic tumor resection.Methods Patients with liver tumors were enrolled from the hepatic surgery department of our hospital.Preoperative ultrasonography (POUS), CT, and intraoperative ultrasonography (IOUS) were performed on the patients respectively.According to the postoperative pathological biopsy, the detection rates of liver tumors in different diameter ranges were compared among the three groups, and the χ2 test was performed to screen out the examination with the highest specificity for liver tumor.Results The detection rate of liver tumors by intraoperative ultrasound was 98.7%, significantly higher than the rates by preoperative ultrasound (80.2%) and preoperative CT examination (87.7%).At the same time, the sensitivity of intraoperative ultrasound examination for small lesions was higher than the other two, with significant differences (P<0.05).Conclusion Intraoperative ultrasonography can supplement preoperative information, improve the detection rate of tumors, reduce the recurrence rate of liver tumors, and improve the prognosis of patients with liver tumor resection.

    Diagnostic value of transbronchial frozen lung biopsy in interstitial lung disease

    Objective To investigate the value of bronchial frozen lung biopsy (TBCB) in the diagnosis of interstitial lung disease (ILD).Methods To analysis the clinic, radiological and pathological parameter of five cases with diffuse lung disease diagnosed by TBCB.Results The clinical symptoms of five patients were mainly cough and progressive dyspnea,with a course of disease ranged from 20 days to 120 months (average 38.53 months).A total of 24 biopsy tissues were obtained, with an average of 4-6 pieces per case.The specimen size was 1-40 mm2, average (8.19±7.73) mm2, and the effective number of tissues accounted for 79.17% (19/24).All obtained pathological diagnosis:two cases of connective tissue disease-associated interstitial lung disease, one case of respiratory bronchiolitis with interstitial lung disease, one case of inhalation-related interstitial lung and one case of common interstitial pneumonia respectively.Conclusion TBCB has minor damage, less complications, and easy access to effective lesions.It is an effective method for the pathological diagnosis of ILD.

    Comparative study of percutaneous transhepatic soft choledochoscopy combining with hard choledochoscopy for complicated intrahepatic and extrahepatic bile duct stones

    Objective To investigate the efficacy of percutaneous transhepatic soft choledochoscopy combining with hard choledochoscopy in the treatment of complicated intrahepatic and extrahepatic bile duct stones.Method A total of 54 patients with complicated intrahepatic and extrahepatic bile duct stones were selected from our hospital from January 2016 to June 2018 and were divided into research group (n=25) and control group (n=29) according to random number table.The research group received percutaneous transhepatic soft choledochoscopy combining with hard choledochoscopy, and the control group received percutaneous transhepatic hard choledodcos copy.Operational time, intraoperative blood loss, operation frequency, length of hospital stay, stone removal rate, and complications between the two groups were compared.Results The research group had longer operation time, shorter hospital stay, less intraoperative blood loss, less complications, and higher stone removal rate (P < 0.05) than the control group, but there was no difference in the frequency of operations between the two groups (P>0.05).Conclusion Percutaneous transhepatic soft choledochoscopy combining with hard choledochoscopy for complicated intrahepatic and extrahepatic bile duct stones is effective, with shorter hospital stay, less trauma and operational blood loss, and faster recovery; thus it is worth of clinical promoting.

    Objective To observe the therapeutic effect of bile duct exploration via laparoscopic-assisted selective small incision on extrahepatic bile duct stones.Methods A total of 70 patients with intrahepatic and extrahepatic bile duct stones treated from September 2016 to March 2019 in our hospital were devided into observation group and control group according to different therapies, with 35 cases in each group.The control group received bile duct exploration via laparotomy, and the observation group received bile duct exploration via laparoscopic-assisted selective small incision.The operation conditions, T-tube drainage volume, bile duct pressure, and operational complications were observed.Results No significant differences in gastrointestinal recovery time, T-tube drainage volume and bile duct pressure on postoperational 1d, residual stone, and bile leakage between the two groups were observed.But bleeding volume, gastrointestinal recovery time, hospitalization time, and T-tube drainage and bile duct pressure on postoperational 7d of the observation group were better than those of the control group (P<0.05); the incision infection rate of the observation group was also lower than that of the control group (2.86% vs.14.29%, P<0.05).Conclusion The bile duct exploration via laparoscopic-assisted selective small incision can reduce operational bleeding volume and complications, shorten gastrointestinal recovery time, thus is worthy of clinical promotion.

    Comparison of Nd:YAG laser under contact lens in treatment of after-cataract in terms of un-dilated and dilated pupils

    Objective To compare the effects of Nd:YAG laser under contact lens on after-cataract in terms of un-dilated pupils and dilated pupils.Methods A total of 92 patients with after-cataract (92 eyes) treated from May 2017 to February 2019 were divided into un-dilated pupils group (n=46) and dilated pupils group (n=46) by random number table.The un-dilated pupils group was given laser therapy without contact lens, and dilated pupils group was treated with laser contact lens after dilating pupils.The visual acuity, intraocular pressure, laser treatment time, laser irradiation energy, and complications in two groups were observed.Results There were no statistical differences in the visual acuity and intraocular pressure between the two groups before and after surgery (P>0.05).The difference in the visual acuity of the two groups was statistically significant before and after surgery (P<0.05).The treatment time and laser irradiation energy in the un-dilated pupils group were shorter than those in the dilated pupils group (P<0.05); There was no statistical difference in the incidence of complications between the two groups (P>0.05).Conclusion The effects of Nd:YAG laser under contact lens on after-cataract in terms of un-dilated pupils and dilated pupils are similar.However, un-dilated pupils treatment without laser contact lens can shorten operation time, decrease laser irradiation energy, and not increase risks of complications, which is relatively safe.

    Effects of tamoxifen and anastrazole endocrine therapies for breast canceron bone metabolism

    Objective To observe the effects of tamoxifen and anatrazole endocrine therapies for breast cancer on bone metabolism.Methods A total of 150 patients with breast cancer who received endocrine therapy after radiotherapy were divided into anatrazole (ANA) group (n=75) and tamoxifen (TAM) group (n=75).All patients were treated for 12 months.The serum levels of calcitonin (CT), osteocalcin (BGP), and bone alkaline phosphatase (NBAP) were measured before and after treatment.The bone mineral density of L2-L4 was measured by X-ray absorptiometry.Results After treatment, the CT and NBAP of the two groups were higher than those before treatment, while the BGP and L2-L4 bone mineral density were lower (all P<0.01 or P<0.05).After treatment, the levels of CT, BGP, and NBAP in the ANA group were higher than those in the TAM group, while the L2-L4 bone mineral density was lower (P<0.01).Conclusion Both the endocrine therapies using TAM and ANA have significant effects on the levels of bone metabolism, especially ANA.

    Clinical analysis of 117 cases of thyroglossal cyst treated by surgery

    Objective To explore the surgical method of thyroglossal cyst and summarize the experience.Methods A retrospective analysis was performed on the clinical data of patients with thyroglossal cyst admitted to our hospital from January 1998 to June 2018.Results Of the 117 patients who received surgical treatment, 110 were cured by one operation and 7 were cured by another operation after recurrence.Postoperative bleeding occurred in 2 cases without complications such as incision infection, pharyngeal fistula and hypoglossal nerve injury.Conclusion Operation is the main method for the treatment of thyroglossal cyst.In order to reduce postoperative complications and recurrence, we should remove part of the hyoid bone and its surrounding suspicious lesions, suture and root pedicle, and completely stop bleeding.

    Clinical significance of sports medicine ultrasound examination for anterior talofibular ligament patients at different rehabilitation stages

    Objective To analyze the clinical significance of sports medicine ultrasound examination for anterior talofibular ligament (ATFL) patients at different rehabilitation stages.Methods A total of 72 ATFL patients treated from Juanuary 2018 to May 2019 in our hospital were selected as study subjects and divided into control group (n=36, without ultrasonography) and observation group (n=36, with ultrasonography).The rehabilitation effect and time, and ligament injury grading after 1, 2, and 3 months of treatment were observed.Results The complete recovery rate in the observation group (91.67%) was significantly higher than that in the control group (77.78%).The rehabilitation time in the observation group [(2.36±0.29) month was significantly shorter than that in the control group [(3.58±0.42) months] (P<0.05).After 1, 2, and 3 months of treatment, the ligament injury gradings in the observation group were better than those in the control group (P<0.05).Conclusion Sports medicine ultrasound examination can effectively judge injury degree, offer important evidence for clinical treatment schemes and rehabilitation plans to shorten rehabilitation time and promote ankle joint function recovery.

    Objective To investigate the efficacy of Exercise-based cardiac rehabilitation on refractory ventricular arrhythmia caused by myocarditis.Methods One patient with myocarditis was treated in the Department of Cardiothoracic Surgery, Tongji Hospital Affiliated to Tongji University and the Department of Cardiology, Kunshan Jinxi People’s Hospital.The patient had frequent ventricular arrhythmias, II atrioventrical block with jaundice and abnormal liver function.Treatment by drug failed.The patient received eight years of individual guidance on cardiac rehabilitation exercises in combination with drug.The treatment effect was observed.Results The patient with refractory ventricular arrhythmia were cured following above protocol.Conclusion It is safe and feasible to treat refractory ventricular arrhythmia by individual guidance on cardiac rehabilitation exercises in combination with drug.

    In recent years, with the rapid development of digestive endoscopy, gastrointestinal endoscopy has been transformed into a new discipline that combines diagnosis and treatment, especially for early cancers in the digestive tract.The incidence of gastrointestinal cancers in China is increasing year by year, and the new techniques of endoscopic imaging can clearly show the microscopic lesions on the mucosal surface, such as lesion boundaries, glandular opening and mucosal microvessels.Therefore, the application of new endoscopic imaging technology can further increase the detection rate of early gastrointestinal cancers.This article provide a brief overview of the current endoscopic imaging techniques commonly used in the field of digestive endoscopy in the early diagnosis of early gastrointestinal cancers.

    Insight into training model of thoracic interns in France

    Objective To analyze the effects of targeted nursing on pain, sleep, and recovery of patients with hepatocellular carcinoma after operation through active pain assessment.Methods Sixty-eight patients with hepatocellular carcinoma who underwent surgical treatment in our hospital were selected and randomly divided into two groups using random number table.The control group (n=34) was treated with routine nursing and assessed by active pain assessment.The observation group (n=34) was additionally treated with targeted nursing.Their postoperative pain, sleep, off-bed time, operation time, and infection were compared.Results The scores of resting pain and active pain of the observation group were lower than those of the control group (P<0.05).The sleep quality scores of the two groups after nursing were lower than those before nursing, and the score of the observation group was lower than that of the control group (P<0.05).The first off-bed time of the observation group was shorter than that in the control group (P<0.05).The total infection rate of the observation group was 2.94%, lower than that of the control group (14.71%) (P<0.05).Conclusion Targeted nursing based on active pain assessment is an effective clinical nursing method because it can effectively reduce pain, improve sleep quality, and speed up recovery of patients.

    Objective To explore the application effect of WeChat platform on family nursing of elderly patients with hip fracture.Methods A total of 100 elderly patients with hip fracture treated from May 2018 to March 2019 were enrolled and divided into control group (n=50) and observation group (n=50) according to random number table.The control group was given routine discharge guidance.The observation group was additionally given nursing after discharge using WeChat platform.The satisfaction of discharge guidance, compliance of follow-up visits, understanding of nursing intervention related knowledge, and complications were compared between the two groups.The patient’s self-care ability after intervention was evaluated.Results The complication rate of the observation group at 6 months after discharge (8.00%) was significantly lower than that of the control group (36.00%) (P<0.05).The Barthel index score of the observation group was significantly higher than that of the control group (P<0.05).Moreover, the medication compliance, follow-up compliance, guidance satisfaction, and understanding of knowledge of nursing intervention of the observation group were better than those of the control group (P<0.05).Conclusion Using WeChat platform for follow-up and family nursing of elderly patients with hip fracture can increase their follow-up compliance, reduce the incidence of complications, elevate self-care ability, and effectively improve prognosis and clinical efficacy, which is worthy of popularization and application.

    Nursing interventions for dangerous placenta previa patients with cesarean section using postpartum balloon

    Objective To study the nursing interventions using postpartum balloon for dangerous placenta previa patients with cesarean section.Methods A total of 78 pregnant women with dangerous placenta previa who underwent postpartum balloon treatment during cesarean section from March 2016 to December 2018 in our hospital were selected and randomly assigned to two groups, 39 cases in each group.The control group was given conventional nursing; and the research group was additionally given comprehensive nursing interventions.The clinical indicators, postpartum complication rate, anxiety and depression scores, and nursing satisfaction were compared between the two groups.Results Compared with control group, the research group had shorter operation time and indwelling time of balloon, and lower postpartum bleeding volume (P<0.05).The complication rates in the research group and the control group were 2.56% and 12.82%, respectively (P<0.05).Before intervention, the anxiety and depression scores between the two groups were not significantly different; after intervention, the score in the research group were lower than that in the control group (P<0.05).The nursing satisfaction rates in the research group and control group were 97.44% and 84.62%, respectively (P<0.05).Conclusion Postpartum balloon is ideal for the treatment of dangerous placenta previa.Through combining comprehensive nursing interventions, the operation time, postpartum bleeding volume, and complication rate are reduced, and nursing satisfaction is increased, indicating that the combined method is worthy of clinical application.

    Effects of clinical confidence nursing on surgical cooperation and preoperative status of patients undergoing elective hysteromyoma surgery

    Objective To explore the effects of clinical confidence nursing on surgical cooperation and preoperative status of patients with elective uterine myoma.Methods From January 2017 to January 2019, a total of 100 patients undergoing hysteromyoma surgery were selected and divided into two groups according to different nursing schemes: a reference group and a study group, with 50 patients in each group.All the subjects were treated with routine nursing, and the study group was additionally given clinical confidence nursing.The operative cooperation and preoperative status of the two groups were compared.Results The systolic blood pressure, heart rate, fasting blood glucose, and score of adverse emotion in the study group were lower than those in the control group before operation (P<0.05), and the degrees of surgical cooperation and nursing satisfaction in the study group were higher than those in the control group (P<0.05).Conclusion Clinical confidence nursing can regulate the tension of patients undergoing hysteromyoma surgery, so as to ensure that the patients can actively cope with the operation, improve surgical cooperation, and reduce stress response to the operation.
