    Effects of pancreaticoenterostomy by two-point method in pancreaticoduo-denectomy

    Objective To investigate the effects of pancreaticojejunostomy by two-point method in pancreatico-duodenectomy.Methods From November 2017 to March 2019, 60 patients undergoing pancreaticoduodenectomy were selected and retrospectively studied.According to surgery methods, the patients were divided into control group (n=30) and observation group (n=30).The control group received bundle-invagination pancreaticojejunostomy and the observation group received pancreaticoenterostomy by two-point method.The surgical conditions and the incidence of pancreatic fistula were compared between the two groups.Results The intraoperative blood loss, time of pancreaticoenteric anastomosis, and hospitalization time of the observation group were significantly less than those of the control group (P<0.05).The total incidence rate of pancreatic fistula of the observation group was significantly lower than that of the control group (P<0.05).Conclusion Pancreaticoenterostomy by two-point method can effectively shorten the time of pancreaticoenteric anastomosis and hospitalization, reduce the incidence of pancreatic fistula with less bleeding, thus has high application value in pancreaticoduodenectomy.

    Comparative analysis of clinical efficacy of two surgical methods for complicated anal fistula

    Objective To analyze the clinical efficacy of two surgical treatments for complex anal fistula.Methods Eighty patients with complicated anal fistula who were admitted to our hospital from September 2014 to March 2019 were enrolled.According to the different surgical methods, they were divided into reference group and observation group, with 40 cases in each group.Traditional incision and suture technique was used in the reference group, and sphincter fistula ligation was performed in the observation group.Clinical treatment effect, intraoperative blood loss, wound area, operation time, wound healing time, anal function, and complications were compared between the two groups.Results There was no significant difference in the total effective rate between the two groups (P>0.05).The amount of blood loss, wound area, and wound healing time in the observation group were significantly less than those in the reference group (P<0.05).After 6 months, the score of anal function and the complication rate in the observation group were lower than those in the reference group (P<0.05).Conclusion In the treatment of complex anal fistula, sphincter fistula ligation is better than traditional incision and suture because it shortens the time of wound healing and has higher clinical value.Therefore, it is worthy of promotion.

    Clinical effects of phacoemulsification plus goniosynechialysis on acute angle-closure glaucoma complicated with cataract

    Objective To investigate the effects of phacoemulsification plus goniosynechialysis on acute angle-closure glaucoma complicated with cataract.Methods From January 2017 to October 2018, 60 cases (60 eyes) of acute angle-closure glaucoma complicated with cataract were selected in the study, received phacoemulsification plus goniosynechialysis, and were followed up for half a year to observe treatment effects.Results The corrected visual acuity, intraocular pressure, and anterior chamber depth after treatment were superior to those before treatment (P<0.05).The chamber angle after treatment was wider than before (P<0.05).There was no severe complication.Conclusion Phacoemulsification plus goniosynechialysis is safe and effective in the treatment of acute angle-closure glaucoma complicated with cataract, and is worthy of clinical promotion.

    Comparison between ultrasound-guided interscalene approach and axillary artery approach of brachial plexus block in distal radius fracture surgery

    Objective To compare the effects of ultrasound-guided interscalene approach and axillary artery approach of brachial plexus block in distal radius fracture surgery.Methods A total of 106 patients undergoing distal radius fracture surgery in our hospital from January to December 2018 were selected and divided into two groups by random number table, with 53 cases in each group.All selected cases received brachial plexus block, the observation group was given ultrasound-guided axillary artery approach, and the control group was given interscalene approach.The disappearance time of neuralgia, anesthetic effect, and complications in the two groups were observed.Results The neuralgia disappearance time of medial cutaneous nerve of forearm, median nerve, and ulnar nerve in the observation group [(10.18±4.32) min, (13.50±7.06) min, and (8.13±4.27) min] were shorter than the time of the control group [(18.04±6.79) min, (19.72±8.14) min, and (21.16±6.39) min]; the rate of successful anesthesia in the observation group (90.57%) was higher than that in the control group (69.81%); and the rate of anesthesia failure in the observation group (3.77%) was lower than that in the control group (15.09%) (P<0.05).There were no statistical differences in the neuralgia disappearance time of lateral cutaneous nerve of forearm and musculospiral nerve and the incidence rate of complications between the two groups (P>0.05).Conclusion Ultrasound-guided axillary artery approach of brachial plexus block for patients with distal radius fracture surgery produces less complications and good anesthetic effect, which can increase the effect of nerve block.

    Objective To investigate the value of preoperative axillary ultrasound combined with methylene blue tracer to show lymphatic drainage in sentinel lymph node biopsy (SLNB) of breast cancer patients with clinically negative axillary lymph nodes.Methods From April 2015 to March 2019, breast cancer patients with clinically negative axillary lymph nodes (n=36) were selected as observation group, and were treated with SLNB by methylene blue staining method and axillary ultrasound before operation to mark lymph nodes at low level and near the breast; the intraoperative sentinel lymph node location and pathological Results were analyzed.Another 36 breast cancer patients were selected as control group during the same period to perform SLNB by methylene blue staining method, and no lymph node ultrasound analysis and body surface labeling were performed before the operation.The detection rate, accuracy, specificity, and sensitivity between the two groups were compared.Results The detection rate of sentinel lymph node in the observation group (97.22%) was higher than that in the control group (72.22%) (P<0.05).The accuracy, sensitivity, and specificity of axillary lymph nodes detected by preoperative axillary ultrasound combined with methylene blue were 94.44%, 96.43%, and 77.78%, respectively.Conclusion Preoperative axillary ultrasound can locate the sentinel lymph nodes, accurately determine the status of axillary lymph node metastasis in breast cancer, reduce the defect of partial absence of blue staining with methylene blue tracer alone, and reduce the incidence of false negative, which is suitable for promotion and application in primary hospitals.

    Application of triamcinolone gelatin sponge in nasal endoscopic surgery

    Objective To investigate the effect of triamcinolone gelatin sponge packing in patients with nasal endoscopic surgery for chronic sinusitis and nasal polyps.Methods The clinical data of 86 patients with nasal endoscopic surgery for chronic sinusitis and nasal polyps from April 2016 to October 2018 were retrospectively analyzed.They were divided into control group (conventional treatment, 43 cases) and observation group (triamcinolone gelatin sponge packing, 43 cases) according to postoperative treatment.The curative effects, bleeding volume, treatment time, pain degree after treatment for 1h, and postoperative adverse reactions were compared between the two groups.ResultsThe treatment effective rate of the observation group (97.67%) was higher than that of the control group (81.40%) (P<0.05).The bleeding volume and the treatment time of the observation group were less than those of the control group (P<0.05).After treatment for 1h, the pain degree of the observation group was lower than that of the control group (P<0.05).The incidence rate of postoperative adverse reactions in the observation group (2.33%) was lower than that in the control group (18.60%) (P<0.05).Conclusion Triamcinolone gelatin sponge packing for patients with nasal endoscopic surgery for chronic sinusitis and nasal polyps can reduce bleeding volume, shorten treatment time, and produce slight pain, which is relatively safe.

    Objective To investigate the clinical effect of high ligation of great saphenous vein plus microwave ablation combined with foam sclerotherapy on varicose veins of lower extremities.Methods From December 2016 to December 2018, 100 patients with varicose veins of lower extremities were randomly divided into conventional treatment group (50 cases) and combined treatment group (50 cases) by random number table.The conventional treatment group was treated with traditional surgery, and the combined treatment group was treated with high ligation of great saphenous vein plus intracavitary microwave ablation combined with foam sclerotherapy.The operation time, hospitalization time, intraoperative blood loss, postoperative complications, and therapeutic effect were compared between the two groups.Results The combined treatment group showed less operation time, hospitalization time, and intraoperative blood loss than the conventional treatment group (P<0.05).The incidence rate of postoperative complications in the combined treatment group (4.00%) was lower than that in the conventional treatment group (24.00%, P<0.05).The total effective rate in the combined treatment group (98.00%) was higher than that in the conventional treatment group (82.00%, P<0.05).Conclusion High ligation of great saphenous vein plus endovenous microwave ablation combined with foam sclerotherapy is effective in the treatment of varicose veins of lower extremities, with fewer postoperative complications, which is worthy of clinical promotion.

    Effect of etomidate combined with remifentanil anesthesia induction on stress and early postoperative cognitive function in senile patients with laparoscopic surgery

    Objective To investigate the effect of etomidate combined with remifentanil anesthesia induction on stress indicators in senile patients with laparoscopic surgery.Methods A total of 72 senile patients with laparoscopic surgery from July 2016 to July 2018 were selected and divided into control group (n=36) and observation group (n=36) by random number table.The control group was given propofol combined with remifentanil anesthesia induction, while the observation group was given etomidate combined with remifentanil anesthesia induction.The concentration of cortisol and the score of Mini-Mental State Examination (MMSE) were compared between the two groups.Results Before intubation, after intubation, 3 min after intubation, and 5 min after intubation, the concentrations of cortisol in the observation group were lower than these in the control group (P<0.05).The postoperative 24 h MMSE score of the observation group was higher than that of the control group (P<0.05).Conclusion Etomidate combined with remifentanil anesthesia induction for senile patients with laparoscopic surgery has better effect of relieving stress response which may be involved in the decline of cognitive function.

    Comparison of two methods to determine the position of left double-lumen endobronchial tubes

    Objective To compare the positioning accuracy of left double-lumen endobronchial tubes by bronchial cuff pressure method and fiberoptic bronchoscopy.Methods A total of 100 patients with single lung ventilation surgery were randomly divided into two groups, with 50 cases in each group.The position of double-lumen endobronchial tubes was assessed by bronchial cuff pressure method and fiberoptic bronchoscopy respectively after double-lumen bronchial intubation.Results The positioning accuracy rate of fiberoptic bronchoscopy (100%) was significantly higher than that of bronchial cuff pressure method (88%, P<0.05).There was no significant difference between the two groups in complications after intubation (P>0.05).Conclusion The bronchial cuff pressure method is simple and practical, with high positioning accuracy and low complications.

    Objective To study the clinical value of extracorporeal shock wave lithotripsy plus ureteroscopic lithotripsy in the treatment of upper ureteral calculi.Methods Fifty upper ureteral calculi patients (diameter of stone:0.9-1.8 cm) treated from January 2005 to April 2019 in our hospital were selected and randomly assigned to two groups, with 25 cases in each group.The study group took extracorporeal shock wave lithotripsy and ureteroscopic lithotripsy; the control group took percutaneous nephrolithotomy.The curative effect was compared.Results The study group showed reduced operation time, intraoperative blood loss, hospitalization time, and hospitalization cost compared with the control group (P<0.05).The stone removal rate in the study group (92.00%) was higher than that in the control group (76.00%, P<0.05); the complication rate in the study group (4.00%) was lower than that in the control group (16.00%, P<0.05).Before surgery, the scores of physiological function, body pain, social function, and role emotional between the two groups were not significantly different (P>0.05).After surgery, the scores in the study group were higher than those in the control group (P<0.05).Conclusion The combination of extracorporeal shock wave lithotripsy and ureteroscopic lithotripsy can increase stone removal rate, shorten treatment time, reduce family financial burden, reduce patients’ complications, and improve their quality of life.

    Prediction and evaluation of postoperative effect of plasmakinetic resection of the prostate in patients with benign prostatic hyperplasia using intravesical prostatic protrusion

    Objective To analyze the value of intravesical prostatic protrusion (IPP) in predicting and evaluating postoperative effect of plasmakinetic resection of the prostate (PKRP) in patients with benign prostatic hyperplasia (BPH).Methods A totoal of 72 patients with BPH in our hospital from May 2016 to November 2018 were selected and divided into significant IPP group (IPP≥5 mm) and non-significant IPP group (IPP<5mm) according to IPP, with 36 cases in each group.All patients underwent PKRP, and the IPP value was measured.International Prostate Symptom Score (IPSS), Prostate specific antigen (PSA), maximum flow rate (Qmax), postvoid residual volume (PVR), and quality of life (QoL) were compared between the two groups.Results Before surgery, there was no statistical difference in the scores of IPSS and QoL between the two groups (P>0.05).After surgery, the scores of IPSS and QoL in the two groups were lower than those before surgery, and the scores in the significant IPP group were lower than the scores in the non-significant IPP group (P<0.05).Before surgery, there was no statistical difference in the levels of Qmax and PVR between the two groups (P>0.05).After surgery, the levels of Qmax were higher and the levels of PVR were lower in the two groups than those before surgery (P<0.05), and there was no statistical difference in the levels of Qmax and PVR between the significant IPP group and the non-significant IPP group (P>0.05).Conclusion IPP can be used as an important index to predict the IPSS and QoL scores in patients with BPH after PKRP.It is easy to operate, repetitive, patient friendly, and non-invasive, thus it is worthy of clinical promotion and application.

    Differential diagnosis between atypical hepatic abscess and hepatocellular carcinoma

    Objective To investigate the differential diagnosis between atypical hepatic abscess and hepatocellular carcinoma.Methods CT reexamination after admission, other tests, and medical history were comprehensively analyzed for differential diagnosis in seven patients newly diagnosed with liver cancer.Results Atypical liver abscess patients had a history of fever, effective antibiotic treatment, AFP negative, low hepatitis b virus carrier rate, and low cirrhosis rate.In particular, all the four patients who delayed the CT reexamination showed hive-like change of liver abscess, namely the cluster-like sign, thus avoiding surgical injury.Conclusion In addition to the data of medical history and examination, it is of great significance to delay the reexamination of CT (about two weeks after the initial examination) for the differential diagnosis of undiagnosed liver occupying lesions if the treatment allows.

    Myxopapillary ependymoma with extensive sacral destruction: a clinicopathologic analysis

    Objective To investigate the clinical and pathological features, diagnosis, and differential diagnosis of myxopapillary ependymoma (MPE) with extensive sacral destruction.Methods Retrospective analysis of the clinical and radiological data, histopathological morphology, and immunophenotype was conducted for one case of MPE with extensive sacral destruction.Results The patient, female, 73 years of age, with a history of low backache for one month, was admitted to hospital with lumbosacral injury.MRI image demonstrated lesions involving vertebral bodies of S2-3.Microscopically, the lesion was composed of cuboidal to elongated tumour cells radially arranged in a papillary manner around vascularied stromal cores.In areas without nipples, myxoid matrix material accumulated between tumour cells and hyaline blood vessels.The tumour cells showed no pleomorphism, with low mitotic activity.Immunohistochemically, tumour cells were positive for GFAP, S100, and Vimentin, and negative for EMA and cytokeratin.Ki67 labeling index was <1% positive.Conclusion MPE with extensive sacral destruction is a rare tumor with indolent behaviour.Late recurrence may occur with incomplete resection.Differential diagnosis include chordoma and metastatic carcinoma.

    Effects of two filling methods on masticatory function in children with posterior teeth defects

    Objective To compare the effects of composite resin and glass ionomer cements filling Methods on masticatory function in patients with posterior teeth defects.Methods A total of 72 patients with posterior teeth defects in our hospital from May 2017 to April 2018 were randomly divided into control group and observation group, with 36 cases in each group.The control group was treated by glass ionomer cements filling method, and the observation group was treated by composite resin filling method.The repair effect and masticatory function after posterior tooth defect filling were compared between the two groups.Results The total effective rate of the control group (75.00%) was lower than that of the observation group (94.44%) (P<0.05).Before treatment, there was no statistical difference in masticatory function between the two groups (P>0.05).After treatment for one year, the masticatory frequency of the control group was lower than that of the observation group (P<0.05); the adverse reaction rate of the observation group was lower than that of the control group (P<0.05).Conclusion Composite resin filling for posterior teeth defect has better effects than glass ionomer cements filling, as it can enhance masticatory function and is easy to operate with less adeverse reactions; therefore, it is worthy of clinical application.

    Objective To explore the innovative management mode of first aid and life support equipment and to realize the delicacy management of medical equipment.Methods Through the dynamic link of QR code, we established the cloud platform of medical equipment account and ensured real-time dynamic data acquisition by means of mobile Internet such as Wechat APP.At the same time, we discussed using the intelligent data gateway based on industrial 4.0 and the intelligent monitoring terminal of medical equipment electrical network quality to obtain the real-time operation data and environmental data of first aid and life support equipment.Results By building the cloud platform of medical equipment account based on Medatc (Wechat official account ) system, we dynamically collected the data of equipment operation status, environmental quality, cost, and efficiency, analyzed the Objective indicators such as the number of patients under healthcare equipment inspection, effective operation time, equipment utilization rate, and equipment environmental health degree, and provide visual presentation of the data on the cloud platform in the first place.It could satisfy the regulatory requirements of relevant policies and regulations for the allocation and use of medical equipment, efficiently draw up daily inventory reports of first aid and life support equipment, improve the rate of equipment in good condition, and ensure the safety of patients.The platform provided explicit data for equipment procurement demonstration, decision-making in the management, etc.Conclusion Through mobile Internet and cloud platform, combined with Internet of things (IoT), a dynamic management platform is established for first aid and life support medical equipment, meeting the JCI requirents for management of medical equipment (standard FMS.8) and for medical service quality (accessibility, effectiveness, and timeliness).

    Health technology assessment and application based on large equipment log analysis

    Objective To obtain accurate equipment operation information by analyzing the equipment log files intelligently, and to provide effective evidence for procurement decision-making by HB-HTA method.Methods The log files of CT equipment were collected, and the data of boot-up time, effective time, check-up number, and scan number were analyzed by using Pandas statistical software.Then Pearson analysis was used to analyze the correlation of scan number, boot-up efficiency, and total boot-up time.Results There was only a moderate correlation between the scan number and the total boot-up time, but a weak correlation between the utilization rate and the total boot-up time, and no correlation between the utilization rate and the total scan time.Conclusion Because of long service life and high maintenance rate, the utilization rate and load of the equipment are low.Therefore, when using HB-HTA for procurement demonstration, the Results can be used as evidence for investigation at the hospital level.

    Objective To evaluate the effect of the teaching mode combining reverse courses with PBL teaching in clinical nursing teaching.Methods Twenty urology nurses were selected from January 2016 to December 2017, and divided into a control group receiving traditional teaching model and an experiment group receiving a combined teaching model of reverse courses and PBL teaching to compare their theory test scores and teaching effects.Results The theory test scores and the instructional satisfaction of the experimental group were superior to the control group, and the differences were statistically significant (P<0.05).Conclusion The teaching mode combining reverse courses and PBL teaching can help nurses with their study, and it is worth promoting.

    Objective To reduce the incidence of complications caused by excessive tourniquet pressure by modifying the setting of pneumatic tourniquet pressure in lower limb surgery.Methods Seventy patients whose expected operation time was less than 90 min under general anesthesia were randomly divided into two groups.In the control group, the pressure of pneumatic tourniquet was set at twice the systolic blood pressure measured with a non-invasive cuff on the upper arm of the patient and locked.In the improved group, the pressure of pneumatic tourniquet was set at 130% of the systolic blood pressure measured with a non-invasive cuff on the lower limb of the healthy side and with a tourniquet applied to a specific location on the limb of the affected side and locked.The blood pressure measurements of right upper limb and unaffected lower limb were observed and recorded after anesthesia.Comparisons were made on indicators such as intraoperative hemorrhage, tourniquet shock, skin conditions where tourniquet was applied at the end of the operation, pains where tourniquet was applied on the first day after the operation, and paresthesia of the affected limb.Results There was no significant difference between the two groups in the proportion of patients requiring pressure release (P=0.051) and intraoperative hemorrhage (P>0.05).The skin conditions of the improved group was better than that of the control group (P<0.05).The postoperative pain degree of the improved group was lower than that of the control group (P<0.001).There were no complications such as tourniquet shock and associated paresthesia in both groups.Conclusion Setting pneumatic tourniquet pressure according to the systolic blood pressure of the unaffected lower limb could achieve a good hemostasis during a fast lower limb surgery.

    Shifting traditional nursing intervention into evidence-based enhanced recovery after surgery (ERAS)nursing mode has allowed patients to enjoy high-quality and scientific clinical care.ERAS-based nursing mode can optimize preoperative, intraoperative, and postoperative management procedures for 3D laparoscopic patients, reduce stress response and complications caused by surgery, shorten hospitalization time, save medical expense, ensure patient safety, and improve patient satisfaction.

    Effect of humanistic care on mental state and life quality of patients after gallstone surgery

    Objective To analyze the clinical effect of humanistic care on mental state and quality of life of patients after gallstone surgery.Methods A total of 112 patients underwent gallstone surgery from March 2017 to July 2018 were selected and randomly assigned to a control group and a study group, with 56 cases in each group.The control group was given conventional nursing; the study group was additionally given humanistic care.The mental state and quality of life were evaluated.Results Before intervention, the anxiety and depression scores between the two groups were not significantly different; after intervention, the scores in the study group were lower than the scores in the control group (P<0.05).Before intervention, the score of quality of life between the two groups were not significantly different; after intervention, the score in the study group was higher than the score in the control group (P<0.05).Conclusion Humanistic care is of great clinical importance for patients undergoing gallstone surgery because it can efficiently improve patients’ mental state, therapeutic effect, and quality of life.It is worthy of clinical promotion and application.
