    Portal vein tumor thrombus is a typical clinical manifestation of advanced hepatocellular carcinoma(HCC), and it is also the main cause of death. In recent years, the idea and strategy of diagnosis and treatment of HCC complicated with portal vein tumor thrombosis have changed greatly with the successive use of targeted therapy and immunotherapy, such as the classification and staging of portal vein tumor thrombus, the process of multidisciplinary diagnosis and treatment, and the combination of comprehensive treatment.

    Study on the mechanism of NLRP3/Caspase-1 pathway on osteoblast pyroptosis based on Caspase-1

    Objective To explore the mechanism of nucleotide binding oligomerization domain-like receptor protein 3 (NLRP3)/cysteine aspartate specific proteinase-1(Caspase-1) pathway on osteoblast pyroptosis by regulating the level of Caspase-1. Methods Mouse embryonic MC3T3-E1 osteoblasts were cultured and divided into group A with high sugar treatment, group B with high sugar + Caspase-1 inhibitor treatment, group C with high sugar + Caspase-1 siRNA treatment, and control group. The level of lactate dehydrogenase (LDH) released from osteoblasts was detected by light absorption method, the osteoblast pyroptosis was detected by Hoechst33342/PI staining, and the NLRP3, Caspase-1, interleukin-1β (IL-1β), and IL-18 of osteoblasts were detected by RT-PCR and Western blot respectively. Results The pyroptosis ratio and LDH activity of MC3T3-E1 osteoblasts in group A, B, and C were significantly higher than those in the control group, while those in group B and C were significantly lower than those in group A (P<0.05). The relative expression levels of NLRP3, Caspase-1, IL-1 β, and IL-18 mRNA and protein in group A, B, and C were higher than those in the control group, while the relative expression levels of Caspase-1, IL-1 β, and IL-18 mRNA and protein in group B and C were significantly lower than those in group A (P<0.05). There was no significant difference in the relative expression levels of NLRP3 mRNA and protein among group A, B, and C (P>0.05). Conclusion Inhibition of Caspase-1 inhibits the elevation of IL-1β and IL-18 caused by the elevation of NLRP3 in osteoblasts due to high glucose, thus inhibiting osteoblast pyroptosis.

    Effects of Mammotome breast minimally invasive rotary resection on benign breast tumors and aesthetic appearence

    Objective To investigate the effects of Mammotome breast minimally invasive rotary resection on breast benign tumor and aesthetic appearence. Methods A total of 110 female patients with benign breast tumors admitted from August 2019 to January 2022 were selected as study subjects, and the patients were divided into the Mammotome group and the control group (55 cases each) according to the random number table method. The control group was given traditional breast tumor surgical treatment, and the Mammotome group was given Mammotome breast minimally invasive rotational treatment to observe the treatment effect and aesthetic changes. Results All patients completed the operation, and no serious complications occurred. The postoperative hospital stay and intraoperative blood loss in the Mammotome group were significantly reduced compared with the control group, and the difference in surgical time was not statistically significant (P>0.05). The complication rates of incisional hematoma, incisional infection, and venous thrombosis at 1 month after surgery in the Mammotome group was lower than those in the control group (P<0.05). The aesthetic effect scores such as breast shape, nipple sensation, and pigmentation at 1 month after surgery were higher in the Mammotome group than in the control group (P<0.05). Conclusion Mammotome minimally invasive rotary resection can reduce the intraoperative bleeding and postoperative hospital stay, improve the beauty effects, reduce the occurrence of complications of patients with breast benign tumor, the rate of axillary lymph node metastasis and distant metastasis were decreased.

    A comparative study of percutaneous pedicle screw placement and Wiltse intermuscular approach for the treatment of thoracolumbar spinal fractures

    Objective To compare the short-term effects of percutaneous pedicle screw placement and Wiltse intermuscular approach on thoracolumbar spinal fractures. Methods Sixty patients with thoracolumbar spinal fractures admitted from April 2021 to March 2023 were selected and divided into a control group and an observation group, with 30 patients in each group. The observation group patients were treated with percutaneous pedicle screw placement surgery, while the control group patients were treated with the Wiltse intermuscular approach surgery. The intraoperative bleeding volume, fluoroscopy frequency, surgical duration, and hospital stay of the two groups of patients were compared; the pain levels between the two groups at 12, 24, 48, 72 and 1 week after surgery were compared; the Cobb angle and anterior edge height of the injured vertebra between the two groups before, immediately after, and 6 months after surgery were compared; the spinal functions of two groups of patients at 6 months after surgery were compared. Results Among the clinical indicators, the observation group had more intraoperative bleeding than the control group; the intraoperative fluoroscopy frequency in the observation group was less than that in the control group; the surgical duration in the observation group was less than that in the control group; the hospitalization time in the observation group was shorter than that in the control group; the postoperative pain scores of patients in the observation group were better than those in the control group at all stages (P<0.05); There was no statistically significant difference (P>0.05) in the Cobb angle and the height of the anterior edge of the injured vertebral body in each postoperative stage. There were no complications such as fixation loosening or wound infection of both groups of patients 6 months after surgery (P>0.05). Conclusion In the operation of thoracolumbar spinal fracture, the use of percutaneous pedicle screws and Wiltse intermuscular approach play a good role in the recovery of patients. Wiltse intermuscular approach can shorten the operation time, reduce the number of fluoroscopy, hospital stay, and postoperative pain of patients, and control the amount of surgical bleeding.

    Effect of extracorporeal shock wave lithotripsy on renal calculi and risks factors of surgical failure

    Objective To explore the therapeutic effect of extracorporeal shock wave lithotripsy on renal calculi and the influencing factors of surgical failure. Methods A retrospective analysis was performed on 600 outpatients and inpatients with kidney stones admitted to our hospital from May 2016 to May 2022. All patients received extracorporeal shock wave lithotripsy and success rate was calculated. The observation group (n=476) and the control group (n=124) were divided according to whether the surgery was successful or not. The general data and the operation conditions of patients in the two groups were compared, and the independent factors affecting the failure of extracorporeal shock wave lithotripsy were analyzed. Results The success rate of primary lithotripsy was 79.33% in the 600 patients. The complication rate was 2.33%, and all patients improved significantly after anti-infection treatment. There was no significant difference between the two groups in terms of gender, age, history of stone surgery, average disease duration, hematuria, pain, hydronephrosis, and other clinical symptoms (P>0.05); there was a significant difference between the two groups in terms of body mass index (BMI), stone size, stone location, surgical position, number of impacts, and operator's working years (P<0.05); there was no significant difference between the two groups in terms of surgical area and lithotripsy voltage (P>0.05). The results of regression analysis showed that surgical position, stone size, and number of impacts were independent risk factors for failure of extracorporeal shock wave lithotripsy for kidney stone treatment (P<0.05). Conclusion The success rate of extracorporeal shock wave lithotripsy for renal calculi is 79.33%, and it is safe. The factors associated with the failure of extracorporeal shock wave lithotripsy for renal calculi mainly include the surgical position, the size of the calculi, and the number of shocks. Therefore, clinically, patients with large stones should try to adopt supine position, and increase the number of shocks while ensuring the safety of patients, so as to further improve the success rate of extracorporeal shock wave lithotripsy.

    Objective To investigate the clinical value of diquafosol sodium combined with sodium hyaluronate for patients with dry eye after cataract surgery. Methods In this study, 236 patients with dry eye after cataract surgery, who were treated with diquafosol sodium and sodium hyaluronate in our hospital from January 2022 to January 2023, were selected as the combined group, at the same time, 82 patients with dry eye after cataract surgery treated with sodium hyaluronate alone were selected as the single therapy group, and the therapeutic effects of the two groups were compared and analyzed. Results The total effective rate of the combined group was significantly higher than that of the single therapy group (P<0.05). There was no significant difference in SPEED score, Fluorescent Test (FL), Tear Breakup Time (BUT), and Schirner Test (Slt) between the two groups before treatment (P>0.05). After treatment, the SPEED score, FL, BUT, and Slt in both groups were significantly improved (P<0.05), and the scores of SPEED, FL, BUT, and Slt in the combined group were significantly better than those in the single therapy group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Compared with sodium hyaluronate alone, the combination of sodium hyaluronate and diquafosol sodium can significantly improve the therapeutic effect on dry eye after cataract surgery.

    Relationship between angiotensin Ⅱ and cardiac injury degree in patients with craniocerebral trauma and evaluation of prognosis

    Objective To analyze the relationship between angiotensin Ⅱ(AngⅡ) and the degree of cardiac damage in patients with craniocerebral trauma and its value in the evaluation of prognosis. Methods A total of 93 patients with craniocerebral trauma admitted to our hospital from January 2020 to December 2022 were selected as subjects. According to whether Glasgow coma score (GCS) was ≤8, they were divided into a severe group (32 cases) and a non-severe group (61 cases).The expression levels of AngⅡ and myocardial enzyme profile [troponin (cTnⅠ),creatine kinase MB (CK-MB),aspartate amino transferase (AST), and lactate dehydrogenase (LDH)] were detected on day 1,3, and 7 after injury in all patients,and the relationship between AngⅡ and myocardial enzyme profile was analyzed. The independent predictors of poor prognosis in patients with craniocerebral trauma were analyzed,and the area under receiver operating characteristic (ROC) curve (AUC) was used to evaluate the efficacy of AngⅡ in evaluating poor prognosis. Results The expression levels of AngⅡ in serum of the two groups on day 3 after injury were significantly higher than those on day 1 and day 7 after injury,and the expression levels of AngⅡ in the severe group on day 1, 3, and 7 after injury were higher than those in the non-severe group (P<0.05). The expression levels of cTnⅠ, CK-MB, AST, and LDH in serum of the two groups on day 3 after injury were significantly higher than those on day 1 and day 7 after injury,and the expression levels of cTnⅠ, CK-MB, AST, and LDH in the severe group on day 1, 3, and 7 after injury were higher than those in the non-severe group (P<0.05).Pearson correlation analysis showed that the expression level of AngⅡ in serum was positively correlated with the expression levels of cTnⅠ, CK-MB, AST, and LDH in serum on day 3 after injury (P<0.05). Multivariate logistic regression analysis showed that serum AngⅡ, cTnⅠ, and CK-MB on day 3 after injury were independent predictors of poor prognosis in patients with craniocerebral injury (P<0.05). According to ROC curve analysis, the AUC of serum AngⅡ for predicting poor prognosis on day 3 after injury was 0.940. Conclusion Serum AngⅡ is closely related to the degree of cardiac damage in patients with craniocerebral injury,and has a good efficacy in evaluating poor prognosis,which is expected to provide an important basis for the diagnosis and treatment of this disease.

    Clinical efficacy of small incision negative pressure drainage combined with irrigation on perianal abscess and its impact on patient complications and quality of life

    Objective To explore the clinical effect of small incision negative pressure drainage combined with irrigation treatment on perianal abscess. Methods A total of 100 patients with perianal abscess from January 2021 to June 2021 were selected and divided into a combined group and a conventional group according to the patient's choices, with 50 patients in each group. The combined group received small incision negative pressure drainage combined with irrigation treatment, while the conventional group received incision drainage treatment. Comparisons were made in the treatment outcomes, quality of life scores before and 4 weeks after surgery, and the incidence of complications 6 months after surgery between the two groups of patients. Results The total effective rate of the combined group (94.00%) was higher than that of the conventional group (78.00%) (P<0.05). After surgery, the WHO QOL-BREF scores of both groups of patients were improved compared to before surgery, and the combined group scored higher than the conventional group (P<0.05); The total incidence of complications in the combined group (2.00%) was lower than that in the conventional group (16.00%) (P<0.05). Conclusion Small incision negative pressure drainage combined with irrigation treatment can effectively improve clinical treatment effectiveness, reduce complications, and improve patient quality of life for patients with perianal abscess.

    Effect of dexmedetomidine on sedation and intracranial hypertension of patients with mild to moderate traumatic brain injury after operation

    Objective To explore the sedative effect of dexmedetomidine on patients with mild to moderate traumatic brain injury after operation and on intracranial pressure (ICP). Methods A total of 180 patients with mild to moderate traumatic brain injury meeting designed inclusion criteria were enrolled as study subjects, and were divided into group A (dexmedetomidine), group B (dexmedetomidine + propofol), and group C (midazolam) by the random number table method. The Richmond Agitation Sedation Scale (RASS) score, ICP, changes of cerebral perfusion pressure (CPP), and occurrence of hypotension and bradycardia were recorded before surgery (T1), 1 h (T2), 2 h (T3), 6 h (T4), 12 h (T5), and 24 h (T6) after surgery. Results The RASS scores and Critical-care Pain Observation Tool (CPOT) scores of group A, group B, and group C at T1-T3 were not significantly different (P>0.05). The RASS score and CPOT scores of group A and group B at T4-T6 were significantly lower than those of group C (P>0.05). The proportions of optimal sedation (RASS score at -2 to +1) of group A and group B at T4-T6 were significantly higher than that of group C (P>0.05), the proportions of over sedation (RASS score < -2) of group B at the same time points were higher than those of group A (P>0.05), and the proportions of agitation (RASS score > +1) of group C at the same time points were higher than those of group A and group B (P>0.05). The ICP values of group A and group B at T3-T5 were significantly lower than the values of group C (P>0.05), and no difference in the lCP values were found between group A and group B at all time points. There was no difference in CPP at all time points (P=0.06) and the incidence rates of hypotension and bradycardia (P=0.15) among the three groups. Conclusion Dexmedetomidine produces a safe and reliable sedative effect on patients with mild to moderate traumatic brain injury after operation, and tends to achieve optimal sedation (RASS score at -2 to +1).

    Objective To observe the effect of transcutaneous electrical acupoint stimulation (TEAS) and thoracic paravertebral block on sleep quality, pain, and quality of recovery after surgery in patients undergoing selective video-assisted thoracoscopic surgery. Methods A total of 60 patients undergoing video-assisted thoracoscopic surgery from June 2022 to March 2023 were selected and divided into the TEAS group or the control group by the random table method. Thirty minutes before anesthesia induction TEAS treatment was performed at Feishu, Neiguan, and Shenmen points bilaterally in the TEAS group until the end of surgery. The control group had the acupuncture electrodes on the same acupuncture point before anesthesia induction without electrical stimulus. The two groups of patients received a TPVB with ropivacaine (0.375%, 25 mL) before anesthesia induction. The visual analog scale (VAS) was used to evaluate pain scores after surgery and Pittsburgh Sleep Quality Index (PSQI) was used for evaluating subjective sleep quality. The PSQI scores were recorded at 24 h before operation (T0), 24 h (T1), 48 h (T2), 7 d (T3), and 14 d (T4) after operation. 15-item Quality of Recovery scores and VAS scores were also evaluated at 24 h after operation. Postoperative analgesic pump use and postoperative nausea and vomiting (PONV) were also recorded. Results Compared with the control group, the TEAS group showed significantly lower PSQI scores at T1, T2, and T3 (P<0.05), a higher QoR-15 score at postoperative 24 h (P<0.05), and lower VAS scores at T1, T2, and T3 (P<0.05). The frequency of analgesic pump pressing of the TEAS group was lower than that of the control group. In addition, the PONV score of the TEAS group was lower than that of the control group (P<0.05). There was no difference in other postoperative complications between the two groups (P>0.05). Conclusion TEAS combined with TPVB can relieve postoperative pain and improve quality of recovery of patients after video-assisted thoracoscopic surgery.

    Analgesic observation of dexmedetomidine combined with ropivacaine bilateral subcostal TAP block for open hepatectomy

    Objective To explore the analgesic effect of dexmedetomidine (DEX) combined with ropivacaine (ROP) bilateral subcostal transversal plane (TAP) block on open hepatectomy. Methods A total of 100 patients with open hepatectomy admitted to our hospital from January 2021 to December 2022 were selected as study subjects and randomly divided into ROP group (n=50) and ROP+DEX group (n=50). All patients received endotracheal intubation, followed by a bilateral subcostal block with ultrasound guidance. The ROP group was given 0.375% ropivacaine 40 mL.The ROP+DEX group was given ropivacaine combined with dexmedetomidine (1 g/kg). Afer hepatic resection was completed, intravenous patient controlled analgesia (PCIA) was given before wound suture. Hemodynamics parameters such as heart rate and mean arterial pressure, analgesic effect, and adverse reactions were observed and compared between the two groups. Results The heart rate and mean arterial pressure in the ROP+DEX group were significantly lower than those in the ROP group at 30, 60 and 120 min after surgery (P<0.05). The first complaint of pain in the ROP+DEX group was later than that in the ROP group (P<0.01). The VAS scores of the ROP+DEX group were significantly lower than those of the ROP group at 2 and 6 h after surgery (P<0.01). There was no significant difference in the overall incidence of adverse reactions between the two groups within 24 h after surgery (P>0.05). Conclusion ROP combined with OEX TAP block for open liver resection under both costal margins has obvious analgesic effect, which can significantly prolong the analgesic time and reduce the degree of pain, stabilize circulation parameters, and realize patients' ambulation out of bed earlier, with high safety.

    Objective To explore of the effect of hysteroscopic electrodesiccation combined with Mirena ring on endometrial thickness and sex hormone levels, and to provide multiple options for the treatment of endometrial polyps. Methods A total of 70 patients with endometrial polyps treated in our hospital from January 2019 to January 2022 were enrolled and randomly divided into a control group and an observation group, with 35 patients in each group. The control group was treated with hysteroscopic electrosurgery combined with post-operative oral progesterone capsules, while the observation group was treated with hysteroscopic electrosurgery combined with the Mirena ring. The endometrial thickness, menstrual fluid volume, sex hormone levels, complication rate, and recurrence rate were compared between the two groups before and after treatment. Results The endometrial thickness and menstrual fluid volume of the observation group were significantly lower than those of the control group at the 3rd, 6th, and 12th months after surgery (P<0.05); the sex hormone level of the observation group was better than that of the control group at the 3rd month after surgery (P<0.05); the complication rate and recurrence rate of the observation group were lower than those of the control group at the 12-month follow-up visit(P<0.05). Conclusion Hysteroscopic electrosurgical resection combined with Mirena is effective in reducing endometrial thickness and menstrual volume, improving sexual hormone levels, and reducing complications and recurrence risk.

    Clinical efficacy of uterine strap suture in patients with postpartum hemorrhage after cesarean section and its impact on intraoperative bleeding volume and out-of-bed activity time

    Objective To study and analyze the clinical efficacy of uterine harness suture in the treatment of postpartum hemorrhage after cesarean section and its impact on intraoperative bleeding and ambulatory time. Methods A total of 120 cesarean section women admitted were selected and divided into a control group and an observation group according to treatment methods, with 60 cases in each group. The control group used water sac compression hemostasis, while the observation group used uterine strap suture. The clinical efficacy of two methods in treating postpartum hemorrhage, as well as their impacts on intraoperative bleeding volume and out-of-bed activity time were compared. Results The total effective rate of the observation group (96.66%) was significantly higher than that of the control group (83.33%) (P<0.05). The intraoperative bleeding volume and out-of-bed activity time in the observation group were significantly lower than those in the control group (P<0.05). The incidence of complications in the observation group (3.34%) was significantly lower than that in the control group (16.68%) (P<0.05). Conclusion Uterine strap suture can reduce intraoperative bleeding and shorten the time to get out of bed, and has a good effect on postpartum hemorrhage.

    Clinical effect of digital technology on anterior dental implant restoration and implant root offset

    Objective To investigate the therapeutic effect of digital technology on implant restoration and implant root offset in the anterior dental area of the mouth. Methods From January 2020 to January 2023, 100 patients who underwent dental implant restoration in the upper anterior teeth area were selected as study subjects. They were randomly divided into a control group and an observation group using a random number table method, with 50 cases in each group. The treatment methods used in the control group and the observation group were traditional implant restoration and digital implant restoration, respectively. The total clinical efficacy and implant offset of the two groups were compared. Results The total clinical effective rate of the observation group was 96.00%, which was higher than 82.00% of the control group (P<0.05). The root offset, angle offset, depth offset, and head offset of the observation group were all smaller than those of the control group (P<0.05). Conclusion The use of digital technology in dental implant restoration in the upper anterior teeth area has a significant effect. It can not only effectively improve the treatment effect, but also reduce the deviation of the implant, which can be further promoted in clinical practice.

    Objective To introduce the construction scheme of a visual collaborative management platform for smart hospital life support equipment, effectively achieving efficient allocation and collaborative management and promoting refined management of hospital life support equipment. Methods A cloud-based architecture was adopted to achieve visual collaborative functions such as equipment monitoring, allocation, and borrowing through data collection and analysis. The platform was also tested for practical applications. Results This platform was different from traditional hospital equipment management mode, effectively solving problems such as delayed equipment monitoring and uneven utilization. At the same time, it achieved comprehensive monitoring, collaboration, and visual functions of equipment, improved equipment fault handling efficiency and equipment utilization, and provided new ideas and methods for the development of smart hospitals and cross hospital campus. Conclusion The construction of a visual collaborative management platform for life support equipment in smart hospitals is an important task and has broad promotion and application value. The application of this platform will provide more convenient and efficient solutions for hospital equipment management, further promoting the high-quality development of hospitals.

    Research on sharing strategy of laboratory equipment in public hospitals

    Objective To investigate the current situation of laboratory equipment sharing in public hospitals, and explore the strategy of laboratory equipment sharing in public hospitals. Methods Questionnaire survey and expert interview were used to study the sharing strategy of laboratory equipment in public hospitals. Results There were many problems in the sharing management of laboratory equipment, such as repeated purchase of laboratory equipment, severe privatization, and weak awareness of sharing management. Based on theoretical foundations, measures such as reasonable allocation of laboratory equipment, improvement of sharing platform systems, strengthening the construction of specialized management teams for laboratory equipment, and improvement of performance evaluation systems were proposed to build an information management system for laboratory equipment. Conclusion Sharing of laboratory equipment improves the efficiency of hospital laboratory equipment use, optimize the allocation of scientific and technological resources, enhance the overall management quality level, and promote high-quality innovative development of the hospital.

    Application of quality control circle activities in reducing defect rate of environmental management of multi-drug resistant organisms in EICU

    Objective To explore the effect of quality control circle activities on reducing the defect rate of environmental management of multi-drug resistant organisms (MDRO) in EICU. Methods A quality control circle group was established to conduct activities themed on "reducing the defect rate of environmental management of multi-drug resistant organisms in EICU", formulate countermeasures, and organize implementation based on current situation investigation and cause analysis, aiming to achieve continuous quality improvement. Results The environmental management defect rate of MDRO in EICU decreased from 34.82% before the activity to 10.87% after the activity. The ability of group members to use the quality control circle to find and solve clinical problems was improved. Conclusion The activities of quality control circle can effectively reduce the defect rate of environmental management of MDRO in EICU and enhance the ability of team cooperation.

    Effect of rapid rehabilitation concept combined with acupoint application of evodia rutaecarpa on intestinal function recovery after appendectomy

    Objective To explore the effect of rapid rehabilitation nursing combined with acupoint application of evodia rutaecarpa on the recovery of gastrointestinal function after appendectomy. Methods A total of 60 cases undergoing laparoscopic appendectomy were randomly divided into a study group and a control group with 30 cases each. The control group received routine rapid rehabilitation nursing, and the study group received acupoint application of evodia rutaecarpa on the basis of routine rapid rehabilitation nursing. The recovery of intestinal function and patients'satisfaction with the effect of rapid rehabilitation nursing were compared between the two groups. Results The first postoperative exhaust time of the study group was significantly shorter than that of the control group(P<0.05). The first defecation time of the study group was shorter than that of the observation group(P<0.05). The incidence of postoperative abdominal distension between the two groups (P>0.05) was not different. The satisfaction of patients in the study group was higher than that in the control group (P<0.05). Conclusion Both rapid rehabilitation nursing and rapid rehabilitation nursing combined with acupoint application of evodia rutaecarpa can reduce postoperative abdominal distension, but the combined therapy can further promote gastrointestinal function recovery, accelerate defecation and exhaust, shorten postoperative fasting time, and improve patients' quality of life and nursing satisfaction.

    To summarize the experience of nursing a middle-aged and elderly man with spontaneous lower esophageal rupture and postoperative chest infection. Nursing points primarily involved thoracic cavity irrigation, respiratory tract nursing care, psychological care, nutritional support care, skin care, disease prevention, and health education. The chest infection was effectively treated without other consequences due to tight collaboration between doctors and nurses, as well as the diligent nursing of ICU nurses. The patient was successfully transferred out of ICU and discharged after 9 days of therapy and nursing.
